New Patients:

I offer therapy and medication management appointments through 8pm, Monday – Friday.

Please e-mail info@renbrookcenter.com to request an intake form. You can simply put “Intake Form” in the subject line and send the e-mail.

You will receive the form via e-mail promptly, typically within a few minutes to 1 hour. After you complete the form, you will hear back promptly, in less than 1 day. Please do not book an appointment online until you hear back regarding your completed form and you are notified to do so. Thank you!
